School physicals are required for kindergarten, 2nd, 4th, 7th and 10th grade. Also, a physical is usually required for college entry. Sports physicals are usually required annually. Many other activities require physicals including daycare, camp, scouting and travel.
If you and your child adhere to the well-child schedule of care , you will not need to make separate appointments for these types of physicals. All forms can be handled at the well visit and physical exams are in effect for one year from the day of the physical.
